Senior Care Options

The Fountains is dedicated to providing the highest quality services you need, when you need them. Our caring staff meets with each new resident to help assesses their unique needs, and create a personalized care plan designed to maintain optimal health and independence. Many of our residents are highly independent, but if a need arises we're here to help!

We offer three Levels of Care:

Care Level 1, $300/month
Available for residents who need personal care assistance a few times a week.

Care Level 2, $600/month
Available for residents who need daily personal care assistance.

Care Level 3, $1,200/month
Available for residents who require substantial personal care assistance multiple times per day.

Enhanced Care is available at The Fountains if you or your loved one begins to require more frequent reassurance and redirection, consistent companionship, or memory care services. Featuring 24-hour line of sight staffing plus all of the personal care, amenities, meals and activities offered daily at The Fountains

Residents may be asked to obtain additional care from a private caregiver or home health care provider if they require more extensive help or the help of more than one caregiver at a time.

Personal Care Services

Personal care services include but are not limited to:

• Escort to and from meals
• Daily bed making
• Routine night checks
• Personal laundry
• Bathing, dressing, and grooming assistance
• Incontinence management
• Redirection and reassurance for memory problems
• Mobility assistance to enhance safety and prevent trips/falls
• Medication management
• Coordination of health care needs
• Monitoring of and assistance with oxygen use
• Assistance with braces, circulation aids and other support garments
